Donguibogam 東醫寶鑑

治飮食過多, 胸腹刺痛如神. 雄黃(水飛) 二錢, 巴豆霜一錢. 右和勻, 滴水爲丸梧子大. 滾漿水煮十二丸, 濾入冷漿水沈冷. 以所浸漿水下一丸, 一日十二時盡服之, 以利爲度, 不必盡劑. 《易老》

Translation

Treats food and drink taken to excess with stabbing pain of the chest and abdomen, with spirit-like effect. Realgar (water-refined) 2 don, croton frost 1 don. Mix evenly and make pills with water drops, the size of a firmiana seed. Boil twelve pills in boiling fermented millet water, then strain them into cold fermented millet water and let them sink and cool. Take one pill down with the water they were steeped in, one at each of the twelve hours of the day until all are taken, stopping when purging is obtained — it is not necessary to finish the dose. — Yeokro 易老 (Donguibogam · Naesang)

Clinical commentary 臨床

Twelve pills for twelve hours, one at a time, stopping the moment the bowels open. That dosing scheme is the safety mechanism: a drastic purgative given in twelfths so that it can be halted as soon as it works. 以利爲度, 不必盡劑 — stopping when purging is obtained, not necessary to finish the dose — is the instruction that matters, and it is stated in the same breath as the schedule. **Contains realgar, which is arsenic sulfide, and croton frost, a violent purgative.
