Translation

Treats wheezing panting with inability to lie down. It may be given only where the illness is of three years' standing or more. Arsenolite powder, 1 don; fermented soybeans pounded to a pulp, 1 nyang; lean pork finely cut, 4 nyang. Mix the three, divide into three portions, wrap each in clay bound with paper fibre, dry the clay over heat, then calcine it in a charcoal fire until the blue smoke has all gone off; set it on the ground for a night to let the fire toxin out, and grind it to a powder. Make pills the size of a mung bean with steamed cake soaked in hot water, and give them after food, down with cold clear tea: twenty pills for an adult, seven for a child. All salty things and soups and watery foods are to be avoided. — Uihak Immun 入門 (Donguibogam · Haesu)

Clinical commentary 臨床

**This must not be prepared. The chief drug is arsenolite — arsenic trioxide — and the dose is given to children as well as adults. Arsenic is cumulatively poisonous and carcinogenic, and preparations of exactly this kind have caused fatal poisoning.** The elaborate procedure is the period's attempt at a margin of safety: the arsenic is bound in soybean pulp and pork, sealed in clay, calcined until the blue smoke is gone, then left on the ground overnight to let out the fire toxin. None of it removes the arsenic. The restriction to cases of three years' standing, and the clause that follows in the chapter about arsenic formulas being 刧劑 — robbing prescriptions — show that the authors knew what they were handling. — Eui Young (Justin) Chung, L.Ac., Dipl.
