Two Immortals Powder for Abscess Pain

Iseon-san

한글 이선산 (옹저)  ·  Pinyin Er Xian San

二仙散

癕 is given above as 癰 and 醫鍳 as 醫鑑. The interlinear weight notes, which the transcription gives in ≪≫, are given in parentheses, the corpus reserving 《》 for source labels. Grams are entered for the 未潰 reading, the state the leaf names first, with the full instruction kept in each prep. This is not Zhang Zihe's melon-pedicel emetic registered as iseon-san, nor the Immun throat formula registered as iseon-san-in; the three share a name only, and this one takes the chapter tail.

Translation

Treats abscess pain that cannot be borne. Angelica dahurica (1 nyang if the sore has not yet opened, 5 don if it has already opened) and fritillaria (5 don if the sore has not yet opened, 1 nyang if it has already opened). Cut them up; for each 7 don 5 fun make one packet, and decoct in half wine and half water, and take. — Uigam 醫鑑 (Donguibogam · Ongjeo)

Clinical commentary 臨床

Two medicinals whose weights trade places, and that is the entire formula. Before the sore has opened, angelica dahurica takes the nyang and fritillaria the five don; after it has opened, the two exchange. Nothing else changes. It is the neatest piece of design in the chapter, because it makes the dosage itself carry the diagnosis. The logic is legible. Angelica dahurica is the chapter's drug for drawing a swelling to a head and dispersing what is still closed; fritillaria dissolves and brings down what has turned to pus. So the unopened sore gets more of the one that opens it, the opened sore more of the one that clears what is draining. The half-wine decoction, as everywhere in this chapter, sends the whole to the surface. Read it against Jitongdanggui-tang two leaves above, the 作痛 subsection's other answer, which treats the same unbearable pain by supplementing qi and blood instead. Neither is analgesia in any modern sense — unbearable pain in an abscess usually means pressure in an undrained cavity, and the treatment for that is a blade. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
