Two Raw Drugs Paste

Isaeng-go

한글 이생고  ·  Pinyin Er Sheng Gao

二生膏

The two raw things of the name are 生地黃 and 生薑. 酒糟 (wine lees) has no herb id in the corpus and is not in the ingredient list, though it is a third of the first method’s bulk. The leaf carries the 初刊本 collation pair 桒 : 桑, but 桒 does not occur in this leaf — it belongs to the 活血散 leaf above it, which writes 以桒皮夾㝎.

Translation

Treats fracture of the hand or foot. Fresh rehmannia 1 geun; fresh ginger 4 nyang. Pound them to a mash, add 1 geun of wine lees, dry-fry it hot, wrap it in cloth and press it on the injured place as a hot compress. For torn sinew and damaged bone with pain past bearing, the effect is remarkable. — Gogeum Uigam 醫鑑. ○ For an injured arm whose socket has come out, swollen and painful: pound fresh rehmannia to a mash, spread it on oiled paper, strew a layer of costus root powder over it, spread more rehmannia on top, and stick it on the affected place; by the next day the pain stops. — Seui Deukhyobang 得效. ○ Treats fracture with torn sinew and damaged bone: pound fresh rehmannia and take the juice, mixed with good wine, two or three times a day — best of all. Also pound it to a mash, steam it hot and seal it over the injury; in a month sinew and bone are joined up again. For rehmannia belongs to bone. — Jonghaeng 種杏 (Donguibogam · Jesang)

Clinical commentary 臨床

The name counts the two raw drugs: 生地黃 a geun and 生薑 four nyang, pounded to a mash, mixed into a geun of wine lees, dry-fried hot, wrapped in cloth and held against the injury. Three methods follow in sequence — the hot compress for a broken hand or foot, a rehmannia sandwich on oiled paper with 木香 powder between the layers for a dislocated elbow, and rehmannia juice in wine taken internally while the mash is steamed and bound on. The last clause gives the reasoning outright: 蓋地黃屬骨, because rehmannia belongs to bone. That is doctrine, not pharmacology. 地黃's category assignment is what puts it in a fracture formula; there is nothing in rehmannia that reaches a healing bone. The compress, though, is a real intervention. Moist heat over a closed injury raises local blood flow and relaxes the muscle guarding around it, and a bound hot pack is genuinely comfortable for the deep ache of a healing fracture — which is what 痛不可忍 is describing a few days in, once the limb is still. 明日痛卽止 for a reduced dislocation is plausible for the same unglamorous reason: the pain of a dislocation stops when the joint goes back, and this went on afterwards. What it must not be used on is an open fracture. Wine lees are a live ferment and the mash is not sterile; sealing fermented plant material against exposed bone invites wound infection and, with soil-contaminated material, tetanus. **A fermented mash dressing on an open wound carries a serious infection risk, tetanus among them. This must not be applied to broken skin.** Heat should also not be applied in the first day or two after an acute injury, when cold and elevation are what reduce the swelling. — Eui Young (Justin) Chung, L.Ac., Dipl.
