活血散

Two different formulas named 活血散 stand in this one chapter, and the parent has pinned the ids by section. This is the 1.7 骨折筋斷傷 one from 得效, a single-ingredient mung-bean plaster for a fracture, registered as hwalhyeol-san-goljeol. The other is in 1.2.5 救急方, a twelve-drug powder given internally for a stab or gunshot wound that has opened the belly and let the bowel out (治刀鎗傷腹裂腸出者), registered as hwalhyeol-san-gugeup. They share nothing but the name. The transcription writes 桒皮 and the 二生膏 leaf below carries the 初刊本 collation pair 桒 : 桑 for it; the house correction 桒→桑 is applied and 桑皮 printed.

Donguibogam 東醫寶鑑

治折傷. 菉豆粉炒紫色, 新汲水調成膏, 厚付折傷處, 以桑皮夾定, 其效如神. 一方, 熱酒醋調付. 《得效》

Translation

Treats fracture injury. Mung bean flour dry-fried to a purple colour, stirred with newly drawn water into a paste, applied thickly to the fractured place and splinted fast with mulberry bark — the effect is as if by magic. Another method: stir it with hot wine and vinegar and apply. — Seui Deukhyobang 得效 (Donguibogam · Jesang)

Clinical commentary 臨床

One drug and a splint. Mung bean flour parched to a purple colour, mixed with fresh-drawn water into a thick paste, laid over the break, and bound fast with mulberry bark — or, in the variant, mixed with hot wine and vinegar. 其效如神 is attached to the whole procedure, not to the flour. It is worth being plain about which half of that procedure does the work. Parched bean starch on intact skin is a cooling, drying, slightly astringent paste and nothing more; nothing in it crosses into bone. 以桑皮夾定 is the treatment. Immobilisation is what lets a fracture heal — hold the fragments still and in reasonable alignment and the body builds callus across the gap on its own — and a strip of mulberry bark bound around a starch plaster is a serviceable field splint. Read that way the formula is not a drug claim at all but a casting technique with a vehicle, and the vehicle is cheap, clean and harmless, which is more than can be said for most of what this chapter puts on a wound. What it cannot do is reduce a displaced fracture, hold an unstable one, or tell you that the bone is displaced in the first place. A paste and a bark wrap over an angulated or open fracture leaves it angulated or open. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
