Translation

For cutting off a malaria in a vacuous person this is the one to use, and any and every malaria may be cut off with it. Ginseng, white atractylodes, white poria, Korean angelica, green tangerine peel, magnolia bark, bupleurum, scutellaria, anemarrhena, wine-steeped dichroa root, tsaoko fruit and vinegar-roasted turtle carapace, 8 fun each; cinnamon twig and licorice, 3 fun each. Cut them up, make one packet, add three slices of ginger, two jujubes, one mume plum and seven peach kernels, decoct in water and leave it out overnight in the dew. On the day the paroxysm is due take it at the fifth watch on an empty stomach; re-decoct the dregs and take that in the morning. Mume plum mixed with sugar is used to take the medicine down. Strictly avoid fowl, fish, bean curd, wheat food and hot things. — Hoechun 回春 (Donguibogam · Haehak)

Clinical commentary 臨床

This is the chapter's answer to its own warning. 截瘧法 says that cutting a malaria off empties the middle, that a patient already depleted will be made worse by it, and that where the cutting drug fails 乃氣大虛, 要扶胃氣為本 — the qi is greatly vacuous and the stomach qi must be propped up first. Here the propping and the cutting are done in one packet: Sagunja-tang with angelica for the qi and blood, Sosiho-tang's bupleurum and scutellaria for the alternation, turtle carapace and peach kernel for the 瘧母 at the rib-side, and wine-steeped dichroa root held down to eight fun so that it cuts without emptying. The instructions around the drugs are as careful as the drugs. Decocted, left out in the dew, taken at the fifth watch on an empty stomach before the paroxysm is due; the dregs re-decocted for a second dose in the morning; sugared mume plum to get it down, since dichroa is bitter and makes the patient retch; and a list of foods to avoid afterwards. The dose has been cut and the timing tightened precisely because the patient cannot afford the full method. **Contains dichroa root, which is emetic and hepatotoxic. This must not be prepared. Malaria requires modern antimalarial treatment.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
