Donguibogam 東醫寶鑑

治一切瘡疥. 藜蘆皮三錢, 蚌粉ㆍ鈆粉各一錢半, 雄黃七分, 輕粉一錢. 右爲末, 另用大鯽魚一箇, 入香油煎候熟, 去魚攤冷, 調藥擦瘡. 《得效》

Translation

Treats every kind of sore and scabies. Hellebore bark 3 don, clam shell powder and lead powder 1 don 5 pun each, realgar 7 pun, calomel 1 don. Powder them. Separately take one large crucian carp, put it in sesame oil and fry it until done, remove the fish, spread the oil out to cool, stir the powder into it and rub it on the sore. — Seui Deukhyobang 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

A rubbing powder of five mineral and plant poisons carried in oil. The carrier is the point of the preparation: a large crucian carp is fried in sesame oil, the fish thrown away, and the fish-flavoured oil cooled and used to work the powder into a paste. Nothing about the carp is medicinal in any sense we would recognise — it enriches the oil so that the paste clings to a scratched surface. 疥 is scabies, the mite Sarcoptes scabiei, and a sulphur or a mineral rub is the historically effective treatment for it; this formula has no sulphur and relies on the mercury and the arsenic instead. **Contains calomel, which is mercury, realgar, which is an arsenic sulfide, lead carbonate, and hellebore, which is emetic and cardiotoxic.
