神異膏

A different prescription from the 神異膏 of 癰疽門上, which is registered as sini-go and gives no composition of its own — that leaf refers the reader away to the Miscellaneous Formulas chapter with the interlinear note 方見雜方. This leaf has a full composition, a different indication and a different source, so it is entered separately with the chapter tail. 治同上 points back to the 一掃散 leaf immediately above, whose indication is 一切瘡疥.

Donguibogam 東醫寶鑑

治同上. 全蝎七箇, 皂角二錢, 巴豆(去皮) 七箇, 蛇床子末三錢, 雄黃(另硏) 三錢, 輕粉一字, 淸油一兩, 黃蠟半兩. 右先用全蝎ㆍ皂角ㆍ巴豆, 入油煎變色, 去三味, 入蠟熔化, 放冷, 入雄黃ㆍ蛇床ㆍ輕粉, 和勻成膏. 先以藥水洗瘡拭乾後, 乃上藥擦之, 神效. 《得效》

Translation

Treats the same as the formula above. Whole scorpion 7, soap-bean pod 2 don, croton seed (skins removed) 7 kernels, powdered cnidium seed 3 don, realgar (ground separately) 3 don, calomel one ja, clear sesame oil 1 nyang, beeswax half a nyang. First put the scorpion, soap-bean and croton into the oil and fry them until they change colour, remove the three drugs, add the wax and let it melt, set it aside to cool, then stir in the realgar, cnidium and calomel and mix it evenly into a paste. First wash the sore with a medicated water and wipe it dry, then apply the paste and rub it in. Marvellously effective. — Seui Deukhyobang 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

A fried-oil paste in the classical order: three gross drugs are cooked in the oil until they colour and then thrown out, so that only what the oil has drawn from them remains, and the fine mineral powders go in after the wax has cooled, which keeps the volatile and the heat-labile drugs intact. The scorpion, the soap-bean and the croton are the extracted drugs; the cnidium seed, realgar and calomel are the ones that stay in the finished paste. Cnidium seed (蛇床子) is the one drug here with a plausible claim against scabies, and washing the sore and drying it before dressing is sound wound care. The rest is mineral poison. **Contains calomel, which is mercury, realgar, which is an arsenic sulfide, croton seed, which is violently toxic, and whole scorpion. This must not be prepared.** Croton oil is a severe vesicant in its own right, and mercury and arsenic cross broken skin. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
