Translation

Treats spleen earth vacuity cold gathering into phlegm drool, so that the stool is dry and bound, the urine red and frequent, and sour water is vomited, turning by degrees into stomach reflux and bound bowel. Black atractylodes steeped six days in rice-washing water, the water changed daily, and lotus seed flesh steeped in wine, 4 nyang each: take one gelded pig's stomach, rub it with wall earth and wash it clean, put the atractylodes and lotus seed inside, sew it up with cotton, cook it soft in wine, pound it fine, press it into small cakes and dry them over the fire. Arisaema finely cut, steeped a night in ginger juice and fried with hearth earth, the earth discarded; pinellia washed in hot water, steeped seven days in vinegar and steamed through; tangerine peel fried with hearth earth, the earth discarded; and millet steeped in ginger juice, steamed and baked dry, 4 nyang each. Ginseng, white atractylodes, white poria, magnolia bark, curcuma and sparganium, these last two both vinegar-fried, cubeb, cardamom, round cardamom, fried millet sprout, fried barley sprout and licorice, 1 nyang each; clove, saussurea and aquilaria, 5 don each. Powder them, make pills the size of a paulownia seed with a thin wheat paste, and swallow sixty to seventy with rice water. — Gogeum Uigam 醫鑑 (Donguibogam · Guto)

Clinical commentary 臨床

The longest preparation in the chapter and one of the longest in the book: twenty-odd drugs, four of them processed with hearth earth, two cooked inside a pig's stomach sewn shut, and the whole thing finally bound with wheat paste. The design underneath all that is Yukgunja-tang with sprouts and aromatics added — a spleen-warming, food-moving pill — and the elaboration is about getting it into a stomach that rejects everything. 結腸, bound bowel, paired with 大便燥澁 is the same observation as 嘔吐噎膈反胃宜通大便: in these patients the vomiting and the constipation are one problem seen from two ends. **The pig's stomach and the hearth earth are not materials to reproduce, and the formula contains arisaema and pinellia, both toxic in the raw state and requiring proper processing.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
