Translation

Treats the same as above. The white field-snail shell, calcined; put in borneol, musk and calomel, a small amount each. Powder the above and smear it on mixed with sesame oil. — Immun 入門. ○ For the sore of pent-up semen, a shell long weathered in a stream channel is best. — Deukhyo 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

A calcined snail shell ground up with pinches of borneol, musk and calomel and made into an ointment with sesame oil. Shell again supplies the absorbent mineral bulk, borneol the cooling and anaesthetic note, musk the costly aromatic, and the oil turns a dusting powder into something that will stay on a moist surface. The appended 得效 note is the interesting part — for 妬精瘡 specifically, prefer a shell that has lain long in a stream channel, 溪港中陳久殼. A shell weathered for years is clean, thoroughly leached and brittle enough to calcine evenly, so the preference has a practical basis even if the stated reason is the drug's accumulated age. **Contains calomel, a mercury salt, which is absorbed through ulcerated skin and causes mercury poisoning; the chapter itself records that outcome at 2.2 as a complication of its own mercury treatments. Musk is taken from a CITES-protected species and must not be used. This must not be prepared.** An oily vehicle is also the wrong choice on a sore the section elsewhere insists on keeping dry, which is the one place this leaf argues against its own neighbours. — Eui Young (Justin) Chung, L.Ac., Dipl.
