The drug was unregistered when this entry was first written; the herb table now carries it as 松蘿, with no Latin species assigned, because the Bogam gives nothing that would fix the lichen's species. The transcription writes 䓗白湯; the corpus prints 葱 and the corpus form is given. Two first-edition collation lines, 一 : 三(初刊本) and 用 : 川(初刊本), stand at the foot of page 031a inside this leaf's span, but neither annotates anything in it — the words they mark are the 各一錢一分 and the 用芎 of the 梔子淸肝湯 leaf a page earlier, and they are recorded there. The honey binder and the scallion-white draught carry no weights.

Donguibogam 東醫寶鑑

治瘰癧ㆍ馬刀堅硬, 形瘦潮熱, 兼治癭氣. 神麴 四錢, 海藻ㆍ昆布ㆍ草龍膽ㆍ蛤粉ㆍ通草ㆍ貝母ㆍ白礬(枯)ㆍ眞松蘿 各三錢, 半夏 二錢. 右爲末, 蜜丸菉豆大, 葱白湯下三十丸. 或取末二錢, 溫酒調服. 《入門》

Translation

Treats scrofula and clam-knife sore gone hard, with a wasted frame and tidal fever; it also treats goitre. Medicated leaven 4 don; sargassum, kelp, gentian, clam-shell powder, tetrapanax, fritillaria, calcined alum and true usnea lichen, 3 don each; pinellia 2 don. Powder the above and bind with honey into pills the size of a mung bean; take thirty in scallion-white decoction. Or take 2 don of the powder stirred into warm wine. — Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

The seaweed formula, and the one place in this section where the pharmacology is real. Sargassum and kelp at three don each are both high in iodine, and 癭氣 — the goitre the leaf names as a second indication — is in most historical cases iodine-deficiency goitre, which iodine treats. Medicated leaven leads on weight at four don, with gentian and clam-shell powder, tetrapanax, fritillaria, calcined alum, usnea lichen and a little pinellia behind: a phlegm-and-hardness formula built around two drugs that happen to carry a hormone precursor. The chapter lists it under 通用 and again under 通治, and the indication it is written for is the hard, matted stage — 堅硬 — in a patient already wasting with tidal fever. That patient does not have goitre. 瘰癧 is tuberculous lymphadenitis, and 形瘦潮熱, a wasted frame with afternoon fever, is active tuberculous disease. The two conditions were run together because both present as a swelling in the neck, and the seaweeds that genuinely shrink an iodine-deficient thyroid do nothing at all to a tuberculous node. **Contains pinellia, which is toxic raw and must be properly processed, and enough iodine from the seaweeds to disturb thyroid function; in someone with autonomous thyroid nodules an iodine load can precipitate thyrotoxicosis, and in others it causes hypothyroidism.
