Donguibogam 東醫寶鑑

治腎虛, 不能行水, 以致浮腫. 附子(炮) 二兩, 白茯苓ㆍ澤瀉ㆍ肉桂ㆍ牛膝ㆍ車前子(炒)ㆍ山藥ㆍ山茱萸ㆍ牡丹皮各一兩, 熟地黃五錢. 右爲末, 蜜丸如梧子大. 空心, 米飮下七十丸或百丸. 《丹心》

Translation

Treats kidney vacuity unable to move water, so that oedema results. Aconite (blast-fried) 2 nyang; white poria, alisma, cinnamon bark, achyranthes, plantago seed (stir-fried), dioscorea, cornus and moutan bark 1 nyang each; cooked rehmannia 5 don. Powder them and make honey pills the size of a firmiana seed; take seventy or a hundred on an empty stomach with rice broth. — Dansim 丹心 (Donguibogam · Bujong)

Clinical commentary 臨床

Palmijihwang-hwan reweighted for oedema. In the parent formula cooked rehmannia leads at eight nyang; here it is cut to five don and aconite is raised to two nyang, so what was a yin-and-yang supplement becomes a yang-warming diuretic. Achyranthes and plantago seed are the additions that make it the Jisheng variant, directing the action downward and out through the urine. This is the formula the chapter reaches for when oedema follows chronic depletion rather than an acute cause — the passage at 2.5.12 names it for water swelling that has followed long-standing dysentery. **Contains aconite at 2 nyang, which is toxic.
