加減凉膈散

The remodel and its reasoning are the entry's treasure: Yanggyeok-san minus rhubarb and mirabilite, plus platycodon, licorice doubled — or saposhnikovia added — making it a 'boat-and-oar preparation' (舟楫之劑) that floats upward. Since the hand and foot lesser-Yang Qi both descend past the diaphragm and net the chest, and the triple burner's Qi roams the body's surface with the minister fire, the diaphragm and six channels are the highest of stations — and this drug, buoyant, likewise a preparation of the heights, races upward through the formless to clear them (Yeokro 易老). Donguibogam 東醫寶鑑

退六經之熱, 又治熱在上焦. 連翹 二錢, 甘草 一錢半, 梔子ㆍ黃芩ㆍ桔梗ㆍ薄荷ㆍ竹葉 各五分. 右剉, 作一貼, 水煎服. ○涼膈散, 去大黃ㆍ芒硝, 加桔梗, 倍甘草, 或加防風, 同爲舟楫之劑, 浮而上之, 治胸膈中與六經之熱. 以手足少陽之氣俱下膈絡胸中, 三焦之氣同相火遊行於身之表, 膈與六經乃至高之分, 此藥浮載, 亦至高之劑, 故能於無形之中, 隨高而走, 去胸膈中及六經熱也. 一名桔梗湯. 《正傳》 《易老》

Translation

Retreats the heat of the six channels, and treats heat in the upper burner. Forsythia 2 don; licorice 1.5 don; gardenia, scutellaria, platycodon, mint, and bamboo leaf, 0.5 don each. Cut the above, make one packet, decoct in water and take. — This is Yanggyeok-san with rhubarb and mirabilite removed, platycodon added, and licorice doubled — or saposhnikovia added — together making a boat-and-oar preparation that floats and ascends, treating heat within chest and diaphragm and the six channels. For the Qi of the hand and foot lesser-Yang both descend past the diaphragm to net the chest, and the triple burner's Qi roams the body's surface together with the minister fire: the diaphragm and six channels are the highest of stations, and this drug, buoyantly laden — itself a preparation of the heights — can race through the formless, following the high places, and remove the heat of chest, diaphragm, and the six channels. Clinical commentary 臨床

Pharmacology as naval engineering — strip the original's purgatives, add the herb that rows upward, and the diaphragm-cooler becomes a vessel: the Yeokro's image of a formula floating 'through the formless, following the heights' is the chapter's most beautiful piece of formula-logic.
