Donguibogam 東醫寶鑑

治産後腫滿喘急尿澁. 黑豆(炒) 一兩, 茯神五錢, 琥珀一錢. 右爲末, 烏豆紫蘇葉煎湯, 調下二錢. 《良方》

Translation

Treats swelling and fullness after delivery with hurried panting and rough urination. Black soybean (stir-fried) 1 nyang; poria with pine root 5 don; amber 1 don. Powder them and take 2 don stirred into a decoction of black bean and perilla leaf. — Buin Daejeon Yangbang 良方 (Donguibogam · Buin)

Clinical commentary 臨床

腫滿喘急尿澁 — swelling, panting and scanty urine together — is the triad of heart failure, and the chapter gives it its own section at 1.29.22. Postpartum cardiomyopathy presents exactly this way in the weeks after delivery. Amber at one don among three medicinals is small but deliberate: it frees the water passages and settles the spirit at once, which is what a breathless patient needs. **Breathlessness with swelling after delivery may be peripartum cardiomyopathy or pulmonary oedema and requires urgent cardiac assessment.** — Eui Young (Justin) Chung, L.Ac., Dipl.
