Translation

Treats a major chest bind. Rhubarb, 3 don; mirabilite, 2 don; powdered kansui, 5 fun. Cut up and divide into two packets. For each packet, first decoct the rhubarb down to six parts, add the mirabilite and bring to a boil once or twice more, strain, then stir in the kansui powder and take. Once a free purge is obtained, stop and do not take the rest. — Junggyeong 仲景 (Donguibogam · Han, lower)

Clinical commentary 臨床

Kansui is toxic and the order of operations in the text is doing real work: the rhubarb and mirabilite are decocted, the kansui is stirred in off the heat as a powder, because boiling it would change what it does and raise the dose that reaches the patient. The instruction to stop the moment the bowels open is the safety margin — one dose held in reserve, discarded if the first works. This is Zhongjing's answer to the chest bind that a mild formula will not shift, and the pattern it treats is a surgical one by modern reckoning. Kansui should not be used. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
