Clinical commentary 臨床

The largest formula in the section and the one built mostly of food and plants: sour jujube seed, clam shell powder and roasted licorice at 5 don lead it, with 四君子 (ginseng, red poria, atractylodes, licorice) inside it and ophiopogon, costus and silkworm filling it out. Gold and silver leaf go in at three sheets each and on the outside as the coating. Also called 大安神元. **Gold and silver leaf are inert metals and are harmless** — the same point as in 金箔鎭心丸. Three things in the list are not. **It contains cinnabar, which is mercuric sulfide, at 2 don 5 fun, and three whole scorpions, whose venom content cannot be standardised; the patient is a child and the text gives no dose by weight anywhere. This must not be prepared.** The third is easy to miss. **代赭石 is haematite, an iron ore, and boiling it in vinegar is a step that makes iron soluble. Iron is one of the classic lethal paediatric ingestions** — a small child can be killed by a quantity of iron an adult would absorb without noticing, through corrosive injury to the gut followed by shock and liver failure. A sweetened, gilded pill, given at a dose of one or two with nothing said about size or age, is exactly the presentation that makes that happen. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
