Donguibogam 東醫寶鑑

治中寒ㆍ口噤ㆍ身强直. 附子(炮)ㆍ人參ㆍ白朮ㆍ乾薑(炮)ㆍ甘草(灸) 各一錢. 右剉, 作一貼, 水煎服. 《三因》 ○一方, 加吳茱萸ㆍ肉桂ㆍ當歸ㆍ陳皮ㆍ厚朴各等分, 煎服, 亦名附子理中湯. 《回春》

Translation

Treats direct cold strike with the mouth clenched shut and the body rigid. Blast-fried aconite, ginseng, atractylodes, blast-fried dried ginger and roasted licorice, 1 don each. Cut up, make one packet and decoct in water. — Samin 三因 ○ Another version adds evodia, cinnamon bark, tangkuei, tangerine peel and magnolia bark in equal parts and is also called Aconite Regulate the Middle Decoction. — Hoechun 回春 (Donguibogam · Han, lower)

Clinical commentary 臨床

Regulate the Middle Decoction with aconite, which is the standard escalation when cold has gone past the middle burner and reached the point of collapse. Lockjaw and a rigid body look like tetany or a convulsive illness, and the Bogam reads them instead as cold congealing the sinews — which is a defensible reading of severe hypothermia, where exactly that rigidity appears. All five drugs at the same dose, the two warming and the three supporting, so the formula warms without driving. The Hoechun variant pushes further down and outward with evodia and magnolia bark.
