Donguibogam 東醫寶鑑

治下焦吐食, 朝食暮吐, 暮食朝吐, 大便不通. 附子(炮) 五錢, 巴豆霜一錢, 砒礵半錢. 右硏極細, 熔黃蠟爲丸梧子大, 每一二丸, 冷水送下, 下利爲度. 利後更服<a href='/formulas/onbaek-won'>紫菀丸</a>, 常服一丸, 勿令再閉. 《保命》

Translation

Treats lower-burner vomiting of food, where food taken in the morning is vomited at evening and food taken at evening is vomited in the morning, and the stool does not pass. Blast-fried aconite, 5 don; croton frost, 1 don; arsenolite, half a don. Grind them exceedingly fine and make pills the size of a paulownia seed with melted beeswax; one or two pills each time, given down with cold water, and the measure is that the stool should pass. After it has passed, give Jawon-hwan instead, one pill regularly, so that it does not close again. — Bomyeongjip 保命 (Donguibogam · Guto)

Clinical commentary 臨床

**This must not be prepared. It contains arsenolite — arsenic trioxide — together with croton frost and aconite, and is the most dangerous formula in the chapter.** Arsenic poisoning from exactly such preparations is well documented, and the beeswax coating was meant to slow the release rather than to make it safe. The clinical situation is real enough: vomiting of food taken half a day earlier, with absolute constipation, is a distal obstruction, and in the absence of surgery a violent purgative was the only thing that could be tried. **A patient with this presentation needs imaging and surgical assessment.** — Eui Young (Justin) Chung, L.Ac., Dipl.
