Supplement the Qi and Drain the Construction Decoction

Bogisayeong-tang

한글 보기사영탕  ·  Pinyin Bu Qi Xie Rong Tang

補氣瀉榮湯

The transcription writes 蘓木 for 蘇木 and 虫 for 蟲; the corpus forms are printed. 䖟 is kept, the herb list heading the gadfly 䖟蟲. The gadfly, leech and peach kernel are count items and the musk is 少許, so none of them carries a gram figure. 爲泥 and 爲末 are preparations and are given in parentheses, and 另末 in the method keeps the instruction that four of the drugs are powdered apart and stirred in at the end rather than boiled.

Translation

Treats leprous wind. First lance the sores until the malign Qi is used up, and then give this medicine. Forsythia and cimicifuga 6 fun each; platycodon 5 fun; scutellaria and raw rehmannia 4 fun each; astragalus, sappan wood, coptis, earthworm, whole scorpion and Korean angelica 3 fun each; round cardamom and ginseng 2 fun each; licorice 1 fun 5 ri; poplar resin 1 fun; a little musk; three peach kernels ground to a paste; three gadflies and three leeches, stir-fried, each powdered. Cut the above; setting aside the poplar resin, musk, gadfly and leech, which are powdered separately, make the rest into one packet, decoct in two cups of water and one of wine down to one cup, strain off the dregs, stir in the finely powdered drugs, decoct down to seven tenths, and take it after breakfast and after the midday meal. — Dongwon 東垣 (Donguibogam · Jechang)

Clinical commentary 臨床

A Li Dongyuan formula, and the only leaf in the leprosy section that treats the sore surgically before it treats it with drugs: 先砭瘡上, 令惡氣消盡, 後服此藥 — lance the sores first and let the malign Qi out, and only then give the medicine. The 大風瘡 section pairs it with Hwangi-san for 鼻崩者, the broken nose. The construction of it is in the name. Ginseng, astragalus and licorice supplement the Qi; forsythia, scutellaria, coptis and raw rehmannia clear heat; and sappan wood with leech, gadfly and peach kernel break the blood. Doses are tiny — six fun at the top and one fun at the bottom — but four drugs are powdered separately and stirred into the strained decoction rather than boiled, which keeps the musk and the insects at full strength. Three leeches and three gadflies in a twice-daily decoction is the chapter's strongest statement that it reads the lesions as blood stasis. **Contains whole scorpion, which is toxic, and musk, a CITES-protected material. Leech and gadfly are anticoagulant and are being given to a patient whose sores have just been lanced. This must not be prepared.** The lancing itself, on anaesthetic skin without asepsis, is how leprous ulcers become secondarily infected and limbs are lost. — Eui Young (Justin) Chung, L.Ac., Dipl.
