Clinical commentary 臨床

The best-reasoned of the scabies rubs in this section, and the one that would most nearly have worked. Sulphur, cnidium seed, sophora root, Sichuan pepper and camphor are all parasiticides or antipruritics of real standing, alum is the astringent, and lard is a proper ointment base that both occludes and carries the drugs into the burrows. A sulphur-in-lard rub is, in substance, the sulphur ointment that was the standard scabies treatment in Western medicine until the middle of the twentieth century. The chaulmoogra kernel is the drug with a genuine history against leprosy, used here for its parasiticidal reputation; the realgar and the calomel are the section's habitual minerals and add nothing the sulphur does not do. **Contains calomel, which is mercury, realgar, which is an arsenic sulfide, and chaulmoogra, which is toxic. This must not be prepared as written.** Camphor is also absorbed through skin and is toxic to infants. Strip the three poisons out and what remains is close to a sulphur ointment, which is still a recognised scabicide — but the formula as it stands is not usable. — Eui Young (Justin) Chung, L.Ac., Dipl.
