Translation

Treats stomach vacuity with vomiting for a month and more, taking neither water nor grain, retching at the smell of food and at the smell of the medicine too. Ginseng, white atractylodes, white poria and dry-fried medicated leaven, 1 don each; patchouli, tangerine peel, cardamom and licorice, 5 fun each; old stored rice, 1 hop. Cut up and make one packet. First steep powdered hearth-oven earth in three seung of water taken from a running stream, let it settle clear and draw off one seung and a half; put in the drugs with three slices of ginger and two jujubes, decoct down to seven tenths, strain, and give it slightly cooled, two or three times a day, and the patient will then keep it down without vomiting. Separately decoct old stored rice and have him sip it from time to time, and he recovers entirely. — Gogeum Uigam 醫鑑 (Donguibogam · Guto)

Clinical commentary 臨床

The formula for the worst case in the section — a patient who has vomited for a month and now retches at the smell of the medicine as well as the food — and almost everything about it is designed around that one problem. The drugs are Sagunja-tang with aromatics; the vehicle is water settled off hearth earth, the standard antiemetic vehicle; the dose is small, cooled, and repeated; and the real treatment is the rice water sipped continuously between doses. 聞藥亦嘔, retching at the smell of the medicine too, is a clinical observation that any physician who has treated persistent vomiting will recognize, and the answer the formula gives is to make the medicine taste as much like food as possible. — Eui Young (Justin) Chung, L.Ac., Dipl.
