Translation

The first is the qi cough, the second the fluid cough, the third the dry cough, the fourth the cold cough, the fifth the evil cough; they do not stop day or night, the face and eyes are puffy and swollen, and food and drink will not go down. Cinnamon bark, dried ginger and soap pod, in equal parts. Grind to a powder, make honey pills the size of a paulownia seed, and give fifteen down with warm water. — Hwaje Gukbang 局方 (Donguibogam · Haesu)

Clinical commentary 臨床

Three hot drugs in equal parts for a cough that never stops, in a patient whose face has begun to swell. The swelling is the detail worth keeping: puffiness of the face and eyes with an unceasing cough and loss of appetite is what a modern clinician reads as right heart failure from chronic lung disease, and the chapter's own 水喘 section describes the same picture. **Soap pod is an irritant saponin drug, emetic and purgative in quantity, and this is a historical record rather than a prescription. Facial swelling with a chronic cough needs assessment of the heart and lungs.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
