White Snake Powder

Baeksa-san

한글 백사산  ·  Pinyin Bai She San

白蛇散

The collation line 䓗 : 葱(初刊本) stands in the transcription at the page boundary immediately after this leaf, before the [033a] marker, but 䓗 occurs nowhere in this leaf; the nearest preceding instance is the 嚼䓗白茶淸下 of 立應散 at 032b, and the note belongs to that leaf rather than this one. 糯米飮, glutinous rice gruel, is the vehicle for the dose and is not entered as an ingredient. 白花蛇肉 is the flesh of 白花蛇, registered as baekhwasa.

Translation

Treats scrofula, the 馬刀 sore and the nine fistulas, with alternating chills and fever and pain. Flesh of the white-patterned viper 2 nyang; green tangerine peel and black morning glory seed (the first powder, half raw and half stir-fried) 5 don each; rhinoceros horn 2 don 5 fun. Powder them; to each 1 don stir in 5 fun of calomel and grind it even, and send it down at the fifth watch with glutinous rice gruel. At the 巳 hour foul matter is passed below, and that is the root of the disease. Take it again after ten days, and afterwards use Saseong-san to supplement it, which cuts the root for good. — Uihak Immun 入門 (Donguibogam · Jechang)

Clinical commentary 臨床

The purgative of the scrofula section, built on a viper and a drastic downward drug. 黑丑 as 頭末, the first and strongest powder sieved from the crushed seed, is the engine; the viper flesh and the rhinoceros horn are the chapter's wind-and-toxin pair; and calomel is stirred into each dose at the last moment. The timing is part of the prescription — taken at the fifth watch, before dawn, so that it works by mid-morning, and what comes away in the stool is read as 病根, the root of the disease itself. A second dose ten days later, then Saseong-san to repair the damage. 瘰癧 is tuberculous lymphadenitis, a chain of caseating nodes in the neck that softens, breaks down and drains for months or years. Nothing in this formula reaches the mycobacterium. The foul matter passed at the 巳 hour is the purge working on the gut, not the disease leaving the neck, and the sinus goes on draining. **Contains calomel (輕粉), a mercury compound, taken internally; the chapter elsewhere records mercury poisoning as a complication of its own treatments. It also contains 黑丑 as a drastic purgative, snake flesh, and rhinoceros horn, which is from a CITES-listed species whose trade is prohibited. This must not be prepared.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
