Donguibogam 東醫寶鑑

治同上. 南星(炮) 二錢半, 朱砂ㆍ牛黃ㆍ羌活各一錢二分半, 麝香六分, 蝎梢七枚, 白殭蠶三枚. 右爲末, 棗肉和丸菉豆大, 薄荷湯化兩丸服之. 《直小》

Translation

Treats the same as above. Arisaema (blast-fried) 2 don 5 fun; cinnabar, ox gallstone and notopterygium 1 don 2 fun 5 ri each; musk 6 fun; scorpion tails 7 pieces; silkworm 3 pieces. Powder them, bind with jujube flesh into pills the size of a mung bean, and melt two pills in a mint decoction and take. — Jikso 直小 (Donguibogam · Soa)

Clinical commentary 臨床

Arisaema at the head, at twice the weight of anything else, which makes this the phlegm formula of the pair — 涎盛, the drooling, slobbering child, is what arisaema is reached for. Round it, the same shelf as the leaf above: cinnabar, ox gallstone, musk, scorpion, silkworm, with notopterygium as the wind drug. **Contains arisaema, which is a severe mucosal irritant unless properly processed — raw, it blisters the mouth and throat — and the patient is a child.** **It contains cinnabar, mercuric sulfide, whose injury to a developing nervous system does not reverse; scorpion, a neurotoxic animal venom that cannot be dosed accurately; and ox gallstone and musk, neither of which has an established paediatric dose.
