Clinical commentary 臨床

A pill built on the Insamyangwi-tang frame — atractylodes, pinellia, aged tangerine peel, magnolia bark, red poria and tsaoko — with ginseng dropped out and dichroa put in at the head of the formula alongside the atractylodes. The name keeps the parent's claim while the composition changes the aim: the base is there to hold the centre, and the dichroa is there to break the paroxysm. What the chapter is reaching for here is the patient in whom the thing has gone on for years. Mume plum at forty-nine fruits is the other half of the design, sour and astringing against drugs that are all drying and dispersing, and the ginger-decoction paste and ginger-decoction draught both exist to keep an aromatic pill from turning the stomach. Two or three years of recurring chills and fever is the description of relapsing or inadequately treated malaria, and the digestive wreckage the formula addresses is real whatever the cause. **Contains dichroa, which is emetic and hepatotoxic. This must not be prepared.** Pinellia is toxic raw and must be properly processed. A relapsing periodic fever needs blood films or a rapid antigen test and modern antimalarial treatment; this formula is recorded as history. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
