Interior-Supporting Toxin-Dispersing Drink

Tangnisodok-eum

한글 탁리소독음  ·  Pinyin Tuo Li Xiao Du Yin

托裏消毒飮

The first line ends 又治隂疽不潰發人 and the next begins 參, so 人參 is split across the line break; 不潰發 belongs with the indication and is read as "neither erupts nor bursts". 隂 is printed as 陰, 歛 as 斂 and 癕 as 癰, following the corpus forms.

Translation

Treats an abscess after it has burst, where the original qi is weak and it has long failed to close over; it is a good agent for removing the rotten and bringing on the new. It also treats a yin abscess that neither erupts nor bursts. Ginseng, astragalus, white peony, Korean angelica, white atractylodes, poria, tangerine peel, forsythia and honeysuckle flower, 1 don each; angelica dahurica and licorice, 5 fun each. Cut them up, make one packet, decoct in water and take it. — Immun 入門 (Donguibogam · Ongjeo)

Clinical commentary 臨床

八物湯 — 四君子 and 四物 together, with the 川芎 and 地黃 left out — carrying 連翹 and 金銀花 for the toxin, 陳皮 so that the supplementing does not clog, and 白芷 to lead the whole thing to the surface. It is the fullest of the 托裏消毒 formulas and the one the chapter reaches for when an abscess that has already burst will not granulate. Read it against 托裏消毒散, its near-twin by name in the same chapter. 去腐生新, removing the rotten and bringing on the new, is the section's own phrase for what a chronic wound needs, and the second indication is the mirror image of the first: 陰疽不潰發, a deep cold swelling that never comes up at all, treated by the same logic of pushing from within rather than cutting from without. A wound that will not close with a depleted patient behind it is a real clinical picture, and nutrition and general condition genuinely do govern whether it heals. The formula is not an alternative to debridement, dressing and antibiotics, and 陰疽 that will not localise is the presentation most likely to be a deep abscess, an infected joint or a diabetic foot — all of which need imaging and surgical assessment rather than a decoction. — Eui Young (Justin) Chung, L.Ac., Dipl.
