Small Blood-Moving Powder

Sohwalhyeol-san

한글 소활혈산  ·  Pinyin Xiao Huo Xue San

小活血散

The leaf carries no indication of its own; 1.85.11 痒痛 supplies it in two places — 痘疹出多身痛叫喚及煩躁脹痛宜小活血散, and again 肉腠密宜小活血散 where the pox is held back by close-knit skin. The transcription writes 為, printed here as 爲, the corpus form by 2,572 places to 69. No entry of this name stood in the database, and no 活血散 already registered is this formula.

Translation

White peony, ground to a fine powder; 1 don at a dose, stirred into weak wine and swallowed. — Jeongjeon 正傳 (Donguibogam · Soa)

Clinical commentary 臨床

One drug. White peony root ground fine, a don of the powder at a time in weak wine, for a child whose pox has come out thickly and who cries with the pain of it. The name claims 活血, blood-moving, and the section's reasoning is that the pain comes of blood not reaching the flesh and interstices, or of skin too close-knit for the pox to rise through; peony is the chapter's standing answer to a pain read that way. It is the one leaf among the six of 1.85.11 and 1.85.12 that is swallowed rather than laid on the skin, so the question of what it would do to an intact or a broken pock does not arise — it does nothing to the surface either way. White peony has no analgesic effect that would reach the pain of a confluent eruption. What was actually useful in this section is the instruction beside the formulas, not the formulas: the itching of the pustular stage is real, it is the reason children tore their own pocks open, and tearing them open invited secondary bacterial infection and deepened the pitted scarring that marked survivors for life. Holding a child's hands was worth more than anything in the cup. **The patient is a child, the vehicle is wine, and the leaf gives no dose scaled to age or weight — 1 don of powder, whatever the child's size.** Ethanol in an infant is not a weak drug: the liver clears it slowly, and the characteristic injury is hypoglycaemia with central nervous depression, which can be severe at quantities an adult would not notice. This must not be prepared. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
