Disperse Wind Powder

Seonpung-san

한글 선풍산  ·  Pinyin Xuan Feng San

宣風散

This is not the 宣風散 already registered as `seonpung-san`. That one is the 丹心 formula given at 1.17.1 撮口 and 1.17.2 臍風 for neonatal tetanus, made of twenty-one wine-roasted scorpions with a ja of musk, and it shares no drug, no source and no indication with this leaf. The two are homonyms. The already registered entry keeps the bare id and this leaf takes the sub-section tail — `-heukham`, from 1.85.19 黑陷 — on the 托裏黃芪湯 / `takrihwanggi-tang-bugol` precedent for two formulas of one name inside a single chapter. 黑丑 is the dictionary head form of the drug and resolves to `heukchuk` directly. The transcription writes 黑䧟 and 𤍠; printed 黑陷 and 熱 as the corpus prints them. 兩箇 is a count and carries no grams; the grams on the morning glory row are the 1 nyang of first-milled powder actually used, not the 4 nyang it is milled from.

Translation

Treats the pox blue and dry and gone black and sunken, with vexing thirst, a distended abdomen and panting, and both passages red and difficult — this being heat shut up within — for which it should be given. Black morning glory seed, 4 nyang, of which 1 nyang of the first-milled powder is taken, half raw and half stir-fried; tangerine peel and liquorice, 2 don 5 fun each; areca nut, two of them. Powder the above finely. A child of two or three years takes 5 fun; a child of four or five years and upward takes 1 don, taken down in a honey decoction. — Jeonssi 錢氏 (Donguibogam · Soa)

Clinical commentary 臨床

Black morning glory seed is the formula; the other three drugs are there to carry it and to blunt it. Four nyang of the seed, half of it raw and half stir-fried, is milled and only the first nyang of powder off the mill is kept — 頭末, the head powder, which is the resin-rich fraction and the strongest. Tangerine peel and liquorice at 2 don 5 fun each and two areca nuts complete it, and it goes down in honey water. The indication is reasoned and the reasoning is stated: the pox has gone blue, dry and black, the child is thirsty and panting with a distended belly, and neither stool nor urine will pass — 乃熱蓄於內, the heat is shut up inside. So open the bowel and the heat goes with it. The signs described are real ones. A blue-dry, flat, dusky eruption is haemorrhagic smallpox; thirst and abdominal distension with panting in a child with a confluent eruption are the picture of a child in serious trouble. The treatment is the wrong way round. **黑丑 is the black seed of Pharbitis, and its purgative principle is pharbitin, a resin glycoside that produces copious watery stools.** The child in this leaf is thirsty, which means dry, and the thing that killed many children with a dense eruption was fluid loss. A drastic cathartic given on top of that makes the one problem modern care would have solved with fluid alone considerably worse. 檳榔 adds arecoline, a cholinergic alkaloid that drives the gut in the same direction; areca nut is also a recognized human carcinogen, which is beside the point for a single dose but worth knowing about the material. **This leaf does scale its dose to the child’s age** — 二三歲兒服五分, 四五歲已上兒一錢, 5 fun at two or three years against 1 don from four or five upward. That is unusual in this chapter and it should be credited: the leaf knew that a smaller child needs less. **But age is not weight.** Children of the same age differ several-fold in mass, a child ill enough to have a black-sunken eruption has lost weight, and a scale with two steps in it on either side of a four-fold difference in dose leaves no margin at all. Scaling by age is what makes this particular leaf worse rather than better: it is a graduated dose of a drastic purgative, handed to a child whose own indication includes 腹脹而喘, a distended abdomen and panting. **Contains 黑丑, black morning glory seed, a drastic purgative, given by mouth to an infant or small child who may already be severely dehydrated. This must not be prepared or given.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
