Augmented Disperse Wind Powder

Gamiseonpung-san

한글 가미선풍산  ·  Pinyin Jia Wei Xuan Feng San

加味宣風散

A 即…加 leaf: the composition is 宣風散 at this sub-section with 2 don 5 fun of 靑皮 added, and the base weights are carried over from that parent leaf, following the `geopungdodam-tang` precedent. The base is this chapter’s 1.85.19 宣風散, registered here as `seonpung-san-heukham`, not the 臍風 formula that holds the bare id `seonpung-san`. 厚朴, 木香 and 糯米 belong to the 四君子湯 taken afterwards, not to this powder, and are left out of `ingredients`. The transcription writes 靑 and it is printed as written, matching the corpus.

Translation

Treats the same as above. That is, the Disperse Wind Powder with 2 don 5 fun of green tangerine peel added. Give it by the method above: black stool is passed first, then brown stool, after which Four Gentlemen Decoction — the formula is given in the Qi chapter — with magnolia bark, costus and glutinous rice added is decocted and taken to settle the stomach; after a good while the stool turns yellow and the sores come out a little of themselves. Then coriander wine — the formula is given above — is sprayed on the body, and the eruption rises at once. — Deukhyo 得效 (Donguibogam · Soa)

Clinical commentary 臨床

The preceding leaf with one drug added. Green tangerine peel, 2 don 5 fun, goes into 宣風散 and the method is unchanged; this is also one of the two formulas 百祥丸 names when it declares itself too drastic, so the chapter treats it as the safer of the purges for black sinking. What is worth reading closely is the rest of the leaf, which is a protocol rather than a prescription. The purge is expected to bring black stool first, then brown; then the child is given 四君子湯 with magnolia bark, costus and glutinous rice to settle the stomach after it; after a good while the stool turns yellow, and that is read as the end point, at which the sores begin to come out on their own; and only then is coriander wine sprayed on the skin to bring the eruption up. A sequence, with a stated marker for when to stop and a restorative built into it. **The marker is almost certainly being misread.** Black stool in a child with haemorrhagic smallpox is melaena — digested blood from the upper gut — and bleeding into the gut was one of the things the haemorrhagic form did; the chapter names the haemorrhagic turn separately at 1.85.15 失血. Here the black stool is taken as proof that the purge is working and the poison is coming out, and the brown and then yellow stool as proof that it has finished. A drastic cathartic given to a child who is bleeding internally will make the bleeding worse, and the sign the leaf is waiting for is the sign that it has. **依上法服之 carries the parent leaf’s dosing across with everything else, so this powder too is dosed by the child’s age** — 5 fun at two or three years, 1 don from four or five upward. The same judgement applies: the chapter is scaling, which is more than it usually does, but age is not weight, and a dose graded in two steps gives no margin to a child who has been purged until the stool ran black. **Contains 黑丑, black morning glory seed, a drastic purgative, given by mouth to a small child who may be dehydrated and bleeding into the gut. This must not be prepared or given.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
