Rhinoceros Horn Toxin-Transforming Elixir

Seogakhwadok-dan

한글 서각화독단  ·  Pinyin Xi Jiao Hua Du Dan

犀角化毒丹

The transcription writes 觧, 為, 䑕 and 熖硝; these are printed 解, 爲, 鼠 and 焰硝. For 熖硝 the corpus prints 焰硝 in 29 places and 熖硝 in none, which is the reading already settled at uhwang-san and chodam-go in this chapter. 䑕粘子 is entered under ubangja by the alias 鼠粘子 the corpus already carries, the same drug the neighbouring sodok-eum-soa and seogaksodok-eum-soa use. The honey binder and the mint decoction are entered with prep and no weight; 薄荷湯 is the draught the pill goes down in and so takes no cross-reference link, following the rule that applies to 甘草湯調服 and 烏梅湯下五十丸. Translation

Treats the residual toxin of the pox and rash not yet resolved, with abscesses and boils breaking out over the head, face and body, or the lips and mouth swollen and broken with sores, the gums bleeding and the breath foul. Platycodon, 1 nyang; forsythia and scrophularia, 6 don of each; dried fresh rehmannia washed in wine, red poria and burdock seed lightly parched, 5 don of each; saltpetre, rhinoceros horn shaved into flakes and liquorice, 3 don of each; natural indigo, 2 don. Make the above into a powder, bind it with honey, and divide each nyang into twenty pills; dissolve one pill in mint decoction and wash it down. — Uigam 醫鑑 (Donguibogam · Soa)

Clinical commentary 臨床

Ten drugs, bound with honey and dosed as a single pill in mint decoction. The weighting is clear: platycodon at 1 nyang, then forsythia and scrophularia, then rehmannia, red poria and burdock seed, with saltpetre, rhinoceros horn and liquorice at the bottom and a little indigo. It is the convalescent formula of 1.87.2 and 1.86 together — the abscesses on one hand, the mouth lesions on the other — and the mouth lesions it names are real: smallpox erupted on the oral and pharyngeal mucosa as well as the skin, and those lesions ulcerated, bled and were the route by which the disease spread by droplet. **Rhinoceros horn is CITES Appendix I. It is keratin — the same protein as hair and fingernail — and has no pharmacological activity of any kind; it cannot clear heat, cool blood or resolve a toxin. The demand this tradition created drove the rhinoceros to the edge of extinction, and it is still doing so. It must not be used.** The 3 don here is a small share of the formula, which makes no difference to either point. **Saltpetre is potassium nitrate, and here it is swallowed rather than applied. Ingested nitrate is reduced to nitrite, which oxidizes haemoglobin to methaemoglobin; infants are the most vulnerable group of all, because fetal haemoglobin oxidizes more readily and the reducing enzyme is not yet mature.** The pill is dosed one at a time, which is the only limit the leaf sets; it gives no dose by weight and no scaling by the child's age. — Eui Young (Justin) Chung, L.Ac., Dipl.
