Translation

Treats phlegm gathered by the seven affects, qi vacuity of the five viscera, and malaria long unresolved. Ginseng, white atractylodes, white poria, aged tangerine peel, pinellia, tsaoko, licorice, mume, fresh ginger and jujube, 1 don each. Cut them up and make one packet; mix in a little salt and let it steep for the time it takes to eat a meal, wrap the packet in skin paper, soak it in water until damp, and roast it over a slow fire until fragrant and cooked through; take it out, decoct in water and take. Several packets taken in succession before the attack comes on are effective at once. — Deukhyo 得效 (Donguibogam · Haehak)

Clinical commentary 臨床

Yukgunja-tang with tsaoko, mume, ginger and jujube added, and all ten drugs weighed the same — which is where the name comes from, four drugs tonifying and four beasts of burden carrying them. Ginger and jujube are counted as drugs here and given a weight, not thrown in as harmonisers. What makes the entry distinctive is the preparation: the packet is salted, left to stand, wrapped in skin paper, damped and roasted in embers until it smells cooked, and only then decocted. The Deukhyo is treating a depleted patient whose phlegm came from the seven affects, so the drugs are softened and warmed before they are ever boiled, and the dose is given in the clear interval before the attack rather than during it. The chapter puts this in the phlegm-malaria section beside Siljin-tang and Naengbu-tang, and it is the one of the three a vacuous patient can take repeatedly without being emptied out. — Eui Young (Justin) Chung, L.Ac., Dipl.
