Donguibogam

Translation

Malaria is nothing other than phlegm repletion with a weak spleen and stomach, lodged in the chest and diaphragm; this is why the dose is taken cold at the fifth watch, so that the medicine reaches downward, strengthens the spleen and stomach, and removes the phlegm repletion. One large aconite accessory root, blast-fried, with the skin and navel removed. Slice it, divide it into two packets, and to each packet add ten slices of ginger; decoct in water, strain off the dregs, leave it out overnight in the dew, and take it cold at the beginning of the fifth watch. — Deukhyo 得效 (Donguibogam · Haehak)

Clinical commentary 臨床

Two drugs, and the whole entry is an argument about route rather than about ingredients. The Deukhyo states its premise first — malaria is phlegm repletion on a weak spleen and stomach, lodged in the chest and diaphragm — and then explains that a hot drug taken cold at the fifth watch will sink past the blockage instead of scattering upward at it. The decoction is strained, left out overnight in the dew to go cold, and swallowed before dawn. It is the mirror image of Nogang-eum, which stands in the same phlegm-malaria section and is also dew-cooled and taken at the fifth watch, but reaches the phlegm by making the patient vomit it rather than by driving it down. **Contains aconite (附子), which is toxic. This must not be prepared.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
