Three Substances Powder

Sammul-san

한글 삼물산  ·  Pinyin San Wu San

三物散

猪頸上毛 and 猫頸上毛, the hair from a pig's and a cat's neck, have no identifiers in the herb list and are left out of the ingredients; they are the two of the three 物 the name counts, so the registered composition is only the remainder. The leaf writes a bare 鼠屎 where the herb list heads the drug 牡鼠屎, and it is registered under that identifier. The calomel added at the end makes a fourth substance against the name's three.

Translation

Treats the soft boil arising at the edge of the sideburn, which is called the sideburn eruption and goes years without healing; this works very well for it. Hair from the back of a pig's neck and hair from the back of a cat's neck, each burnt with its nature preserved, a pinch of each; one rat dropping. Powder the above, add a little calomel, and apply stirred with clear oil. — Deukhyo 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

Pig's neck hair and cat's neck hair, each burnt 燒存性 so that the ash keeps something of the substance, with a single rat dropping and a little calomel, made up in oil. Burnt hair ash appears repeatedly in this chapter and amounts to carbon; the three 物 of the name are the two hairs and the dropping, and the mercury is an addition the title does not count. 發鬢 is the boil at the edge of the sideburn, and the leaf's 有數年不愈 is the whole complaint: a recurring furuncle or folliculitis in the beard and temple area, which in modern terms is nearly always staphylococcal and often sustained by nasal carriage. The site matters — a boil in the mid-face and temple drains toward the cavernous sinus, which is why squeezing one there is a genuine hazard. **Contains calomel, a mercury salt, and must not be prepared. Rat droppings must not be put on an open boil** — rodent faeces carry leptospira, salmonella and hantavirus, and on a broken skin surface in the mid-face this is a serious infection risk rather than a quaint one. There is nothing in the formula that treats the condition. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
