Donguibogam 東醫寶鑑

治孕婦轉胞尿閉. 四物湯<span class='sochu'>方見血門</span>材各一錢, 加人參ㆍ白朮ㆍ半夏ㆍ陳皮各一錢, 甘草五分. 右剉作一貼, 入薑三, 水煎, 飮後探吐之, 又與又吐, 小便立通, 神效. 《丹心》

Translation

Treats a pregnant woman with the bladder turned and the urine blocked. The material of Four Substances Decoction <span class='sochu'>formula in the Blood chapter</span> 1 don each, with ginseng, white atractylodes, pinellia and tangerine peel 1 don each added, and licorice 5 pun. Cut up, make one packet, add 3 slices of ginger, decoct in water; after drinking, probe to induce vomiting, then give again and induce vomiting again, and the urine passes at once, with spirit-like effect. — Dansim 丹心 (Donguibogam · Buin)

Clinical commentary 臨床

Treating urinary retention by making the patient vomit is the chapter's most surprising method, and the reasoning is mechanical: raising the qi sharply lifts the gravid uterus off the bladder neck. 轉胞, the turned bladder, is what modern obstetrics calls uterine incarceration, and relieving it does depend on shifting the uterus. Whether retching achieves that is another matter, but the target is correctly identified. **Urinary retention in pregnancy requires catheterization and assessment.
