Translation

Treats damp heat with urgent panting. The head powder of white morning-glory seed, 2 nyang, half raw and half dry-fried; rhubarb, 7 don; tangerine peel with the white removed, 5 don; unripe tangerine peel and areca, 3 don each. Grind to a fine powder and give 3 don each time stirred into ginger decoction. ○ A woman who was naturally stout, in a spell of long rain, took wine and buttermilk and got abdominal distension with panting fullness; the sound of it was heard outside the house, she could not lie at ease, urine and stool were sluggish and inhibited, and the pulse at the qi opening was twice the size of that at the man's greeting, sunken and moderate but forceful. This was panting from damp heat grown excessive. Where the evil qi is abundant there is repletion, and repletion should be precipitated downward. One dose of this medicine halved it, a second stopped the panting; only fullness of the chest, a dry mouth and an occasional cough remained, and Modified Drain the White Powder given after it cured her entirely. — Wigye Bogam 寶鑑 (Donguibogam · Haesu)

Clinical commentary 臨床

A purgative given for breathlessness, and the case history attached to it shows the reasoning being made rather than asserted: a stout woman, wet weather, wine and buttermilk, distension and audible wheeze, constipation, and a forceful pulse. Everything in that picture says repletion, and 實者宜下之 — repletion should be precipitated downward — is the rule applied. The panting stopped when the bowel opened. 白牽牛頭末 is the first-pressed powder of morning-glory seed, the strongest fraction, and half of it is given raw to make it stronger still. With rhubarb beside it this is a brisk cathartic, and the drugs that remain — the two tangerine peels and areca — are there to move the qi downward with it. What the case describes is most likely cardiac or obesity-related dyspnoea with gross constipation, where relieving the loaded bowel genuinely eases the breathing. **Repeated purgation with morning-glory seed and rhubarb causes fluid and potassium loss; it is recorded here as history.** — Eui Young (Justin) Chung, L.Ac., Dipl.
