Donguibogam 東醫寶鑑

治傷損墮落, 惡血流於脇下, 痛楚不能轉側. 水蛭(炒烟盡, 另硏) 三錢, 柴胡ㆍ連翹ㆍ當歸梢各二錢, 蘇木一錢半, 羌活ㆍ防風ㆍ桂皮各一錢, 麝香少許. 右除水蛭ㆍ麝香外, 餘藥剉作一貼, 酒水相半煎, 去滓, 入蛭ㆍ麝調服. 空心, 兩貼立愈. 《東垣》

Translation

Treats injury from a fall with foul blood running down into the flank below the ribs, the pain so bitter that the patient cannot turn from side to side. Leech, 3 don, stir-fried until the smoke is gone and ground separately; bupleurum, forsythia and angelica sinensis tips, 2 don each; sappanwood, 1 don 5 fun; notopterygium, saposhnikovia and cassia bark, 1 don each; musk, a small amount. Setting aside the leech and the musk, cut the remaining drugs as one packet and decoct in half wine and half water; strain off the dregs, stir in the leech and the musk, and take it. On an empty stomach, two packets and it mends at once. — Dongwon 東垣 (Donguibogam · Jesang)

Clinical commentary 臨床

Li Dongyuan's formula, and the construction is his: bupleurum to reach the flank, where the injury is said to be, and the blood-movers behind it. Leech and musk are kept out of the decoction and stirred into the strained liquid, because boiling would spoil both — a real pharmacological observation. Notopterygium and saposhnikovia are the wind drugs that carry the formula to the surface and the sinews; cassia bark warms the channel. 惡血流於脇下, 痛楚不能轉側 is a good clinical description. A patient who cannot turn over after a fall has a rib fracture, a chest-wall haematoma, or an injury to the spleen or liver beneath. The first two hurt for weeks and get better on their own; the third can kill in hours. A formula that cannot distinguish them is being given blind, and three don of leech — an anticoagulant — is the wrong drug if the bleeding is still going on. **Contains leech as an internal drug and musk, taken from a protected animal.
