Donguibogam 東醫寶鑑

治風, 寒, 濕, 氣, 血壅滯, 臂胛疼痛. 五靈脂ㆍ荊芥穗ㆍ防風ㆍ羌活ㆍ獨活ㆍ穿山甲ㆍ骨碎補ㆍ草烏(製)ㆍ甘草節各五錢, 麝香半錢. 右爲末, 每二錢, 溫酒調, 臨睡服. 《得效》

Translation

Treats wind, cold, damp, Qi and Blood obstructed and stagnant, with aching pain of the arm and scapula. Flying squirrel feces, schizonepeta spikes, siler, notopterygium, aralia, pangolin scale, drynaria, processed kusnezoff monkshood root, and licorice node 5 don each; musk 0.5 don. Powder the above; 2 don each time, mixed with warm wine, taken at bedtime. — Deukhyo 得效 (Donguibogam · Su)

Clinical commentary 臨床

Two constituents cannot be dispensed. Pangolin scale (穿山甲) is listed on CITES Appendix I with all eight species protected under the US Endangered Species Act; musk (麝香) is likewise CITES-listed and not importable. Where the classical intent is to pierce obstruction and open blocked channels, contemporary Korean practice substitutes wangbulryuhaeng (王不留行) or jogakja (皂角刺); for the aromatic penetrating action of musk, seokchangpo (石菖蒲) is the usual stand-in. Neither substitution reproduces the classical action, and the formula is recorded here as documented rather than as prescribable in this form. Note also the processed kusnezoff monkshood — unprocessed 草烏 is acutely cardiotoxic — and the bedtime dosing, which suits a pain that deepens at night. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
