Clinical commentary 臨床

A warm formula throughout — dried ginger and cinnamon at the head of it — with schisandra to gather the lung's qi in and ass-hide gelatin to hold the fluid. The combination of warming, astringing and moistening is what the chapter means by 肺虛客寒, a vacuous lung with a cold lodged in it: the cold has to be dispersed but the lung is too depleted to be dried out. 痰沫, frothy phlegm, in large amounts with breathlessness is the presentation. In a chronic case that is bronchial hypersecretion; in an acute one with a failing heart it is pulmonary oedema, and the two are not distinguished here. **Contains pinellia, toxic in the raw state and to be properly processed, and asarum, which modern pharmacopoeias restrict to smaller doses than the one given here.** — Eui Young (Justin) Chung, L.Ac., Dipl.
