Warm and Resolve Powder

Onhae-san

한글 온해산  ·  Pinyin Wen Jie San

溫解散

The transcription writes 溫觧散 with the variant graph of 解, which appears nowhere in the corpus; the standard form is printed above in the name and the id romanised from it. 官桂 is registered under 肉桂 and 白薑 under 乾薑, both by way of the aliases. 甘草灸 runs the preparation after the drug in the leaf and is parenthesised here. The three slices of ginger and two jujubes are counts and carry no weights.

Clinical commentary 臨床

不換金正氣散 with five drugs added. The base — atractylodes lancea, magnolia bark, tangerine peel, agastache, pinellia and licorice — is the standard aromatic prescription for damp turbidity in the middle, and the 得效 extends it outward with chuanxiong, angelica dahurica and asarum to reach the surface, and cinnamon and dried ginger to warm. Nothing in it addresses the sinus directly; it treats the state of the patient in which a sinus is held to persist. The chapter's own reasoning is set out at the head of the 疳瘻 section and it is worth following, because this is the first prescription it names. 穿孔一深, 膿汁不盡, 風冷倂入, 涓涓而成漏 — the hole goes deep, the pus is never done, wind cold gets in with it, and it trickles until it is a fistula. 治法溫散風冷爲急, warming and dispersing the wind cold is the urgent thing. So warmth and aromatics come first, drying the discharge second, and engendering flesh third; this leaf is step one. A 漏 is a chronic fistula or sinus, and the section's own account of why one persists is better than its treatment: 苟有宿膿朽骨停蓄其間皆爲之漏 — where old pus or dead bone is held in it, it becomes a fistula. That is correct, and it is the reason a decoction cannot close one. A sinus closes when what is feeding it is removed, which means imaging to find the track and any sequestrum, culture, antibiotics and surgical excision of the tract. **Contains asarum at 1 don, which is toxic in quantity and contains aristolochic acid in some commercial material.** — Eui Young (Justin) Chung, L.Ac., Dipl.
