Food-Dispersing Pill for the Five Gan

Ogamsosik-won

한글 오감소식원  ·  Pinyin Wu Gan Xiao Shi Yuan

五疳消食元

The transcription writes 殺䖝 here, 殺虫 in 十味蘆薈丸 and 肥兒丸; all three are normalized to 殺蟲, the form the corpus prints. This leaf writes 山査肉 where 疳積餠, 消疳丸 and 肥兒丸 write 山楂肉, and each leaf keeps its own glyph.

Translation

Treats gan of every kind, killing worms and driving off heat, grinding away accumulation and bringing on the appetite. Quisqualis, gentian root, malt, tangerine peel, elm-fruit paste, medicated leaven, coptis and hawthorn flesh, equal parts of each. Powder the above and bind it with cooked old rice into pills the size of a millet grain; twenty or thirty in rice decoction. — Immun 入門 (Donguibogam · Soa)

Clinical commentary 臨床

Eight drugs in equal parts — the smallest and plainest formula in the 諸疳通治藥 group. Two worm drugs, Quisqualis and elm-fruit paste; three food-movers, malt, medicated leaven and hawthorn flesh; tangerine peel to move the qi; and gentian root with coptis for the heat. Bound with cooked old rice into millet-sized pills, which is a sensible size for an infant and the one concession the leaf makes to its patient. **The two worm drugs are the active part and they work.** Quisqualis is a genuine anthelmintic and is still in use; elm-fruit paste is another. Ascariasis is what they treat, it is common wherever children are malnourished, and a heavy worm burden takes calories from a child who has none to spare. A single dose of albendazole or mebendazole now does the same job at a known dose for a few cents. **An effective drug with no stated dose is still a hazard** — equal parts of nothing in particular, twenty or thirty pills, no weight of child; enough 使君子 causes hiccup, vomiting and diarrhoea. **The patient is a small child and the text gives no dose by weight anywhere in this chapter.** Nothing in these eight drugs is acutely toxic, which makes this one of the safer entries in the group; the failure is the structural one. On 疳 as childhood malnutrition, and on why worm drugs and bitters are not a treatment for a starving child, see 五疳保童元; the one formula of this group that actually delivers calories is 疳積餠. — Eui Young (Justin) Chung, L.Ac., Dipl.
