Translation

Treats sudden turmoil with vomiting and diarrhoea, and cramping and cold reversal of the limbs. Quince, evodia and table salt, 5 don each. Dry-fry them together until scorched, put three seung of thoroughly boiled water in a jar, add the drugs and decoct down to two seung, and let the patient take it hot or cold as he likes; it works at once. If the above drugs are not to be had, give 1 don of calcined alum powder stirred into boiling water. If there is no alum, decoct a pinch of salt with a cup of vinegar and take it. Or salted plum, or anything salty and sour, may be boiled and taken. — Uihak Immun 入門 (Donguibogam · Gwallan)

Clinical commentary 臨床

The chapter's main formula for cramping, and the three drugs are chosen for what cramp needs: quince relaxes the sinews, evodia warms and settles the stomach, salt replaces what has been lost. The substitutions the source offers if the drugs are unavailable all keep one of the two tastes — salty and sour — which is a clue to what was thought to be doing the work. The cramps of 轉筋 after profuse vomiting and diarrhoea are a loss of salt and water, and a salty-sour drink taken warm is a rough oral rehydration solution. That part of the reasoning holds. **Severe cramping with cold limbs after violent vomiting and diarrhoea means serious dehydration and electrolyte derangement; it needs proper rehydration and medical assessment.** — Eui Young (Justin) Chung, L.Ac., Dipl.
