Translation

Treats vomiting and diarrhoea in the newly born, which is caused by filth and foulness entering the stomach. Cinnabar, arisaema and croton frost, equal parts of each. Powder the above and make paste pills the size of millet grains; two or three pills dissolved in a mint decoction, and afterwards regulate the child with Jusa-sinking Preparation. — Immun 入門 (Donguibogam · Soa)

Clinical commentary 臨床

The reasoning is the chapter's own: a newborn that vomits and purges has swallowed the filth of birth, so the filth must be driven down and out, and then the stomach settled. Croton frost does the driving, arisaema the phlegm, cinnabar the settling, and the whole is rolled into millet-sized pills because the patient is days old. That reasoning is why the formula is dangerous rather than merely useless. **This must not be prepared or given.** Cinnabar is mercuric sulfide and the full account of what mercury does to a developing nervous system is on 辰砂膏 in this chapter; what matters here is that the patient is a newborn, whose blood–brain barrier is immature and whose kidneys clear poorly, and that the text gives **no dose by weight anywhere** — 各等分, equal parts, and two or three pills. **Croton frost is a violent purgative and a vesicant**, and this is the sharpest version of the hazard anywhere in the chapter: the child is already losing fluid by both routes, and a drug given deliberately to increase that loss can kill by dehydration alone, with the mercury and the arisaema beside the point. **Raw arisaema is toxic and has to be properly processed**; the leaf writes 南星 bare, with no processing given. The illness is acute gastroenteritis, and what saves such a child is oral rehydration solution. The full statement is on 玉露散, the first leaf of this section. A neonate who is vomiting and passing loose stool is in any case a medical emergency on other grounds — sepsis, obstruction and metabolic disease all present this way in the first weeks — and needs assessment, not a purge. — Eui Young (Justin) Chung, L.Ac., Dipl.
