Regulate Qi and Benefit the Yellow Powder

Jogiikhwang-san

한글 조기익황산  ·  Pinyin Tiao Qi Yi Huang San

調氣益黃散

The goose quill is apparatus and stays in the transcription and the translation. The source label 錢乙 is not in the chapter's standard list and is romanised Jeoneul, the form the corpus already uses for it. The transcription writes the silkworm 白殭蚕 and that form is kept.

Translation

Treats all three signs — clamped mouth, pursed mouth and navel wind. One gold-headed, red-footed centipede (steeped in wine and roasted), four scorpion tails, seven white stiff silkworms (stir-fried), dianthus 5 pun. Grind to a powder and blow 1 ja at a time into the nostrils through a goose quill; if the child sneezes and cries it can be treated. Then give 1 ja by mouth, stirred into a mint decoction. — Jeoneul 錢乙 (Donguibogam · Soa)

Clinical commentary 臨床

Centipede, scorpion tail and stiff silkworm — the three animal wind drugs together — with a little dianthus, blown up the nose and then swallowed. Qian Yi's formula is the one the chapter reaches for first: 1.17.2 臍風 names it as the treatment of choice and keeps 金烏散 and 宣風散 for the severe case. The interesting part is not the drug but the test. 噴嚏啼哭則可治 — if the child sneezes and cries, it can be treated — is prognosis, and read as prognosis it is sound. A newborn in tetanus whose spasm has not yet taken the airway can still draw breath, sneeze and cry; one that cannot do any of those has laryngeal spasm and respiratory failure and will die without ventilation. The chapter has found a crude bedside severity score and framed it as a sign of treatability. Modern triage asks the same question with better tools. For what the condition is, why none of this reaches it, and what cord care actually prevents it, see the note on `heolcho-san-soa`. **Contains centipede and scorpion, both neurotoxic animal venoms, blown into the nose and then given by mouth to a newborn. Neither can be dosed by potency, and the text gives no dose by weight anywhere; in an infant of roughly 250 ml blood volume, with an immature blood–brain barrier and poor renal clearance, that margin does not exist. Blowing a powder into the nostrils of a baby whose airway is already threatened risks aspiration on its own. This must not be prepared or given.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
