Translation

Wine-steamed rhubarb, 4 nyang; pig-tusk gleditsia pods, 2 nyang. Powder them, bind with a wheat-flour paste into pills the size of a mung bean, and take fifty to seventy at a dose, sent down with a mung bean decoction; sweating is taken as the effect. ○ In the bingxu year of the Wanli reign I was lodging at Daliang when an epidemic pestilence broke out greatly. Gentry and commoners died in numbers, to the point of whole households being wiped out. The signs were aversion to cold with vigorous heat, head, face, neck and nape red and swollen, the throat swollen and painful, and a clouded confusion; it was called epidemic swelling of the head. I brought out a secret prescription and named it Iseonggugo-hwan. Gleditsia was used to open the gates and orifices and so issue the exterior, and rhubarb to drain all the fires and so free the interior. One dose and there was sweat; one sweat and there was recovery — truly an immortal’s prescription. In those of a strong constitution it hit a hundred times out of a hundred; for the vacuous and weak, give Hyeongbangpaedok-san first, and if that does not cure, Ubanggeumryeon-tang is also effective. — Hoechun 回春 (Donguibogam · On)

Clinical commentary 臨床

Two drugs, and the leaf explains them itself: pig-tusk gleditsia to open the gates and orifices and push the illness out through the surface, wine-steamed rhubarb at double weight to drain the fires and open the interior. Sweat is the endpoint the author watches for. The mung bean paste and mung bean decoction are not decoration — mung bean was the standard cooling antidote vehicle, and it tempers a pill this harsh. What makes the leaf unusual is that it is a first-person outbreak report. Gong Tingxian puts himself at Daliang in 1586, names the mortality, lists the signs, and then gives his triage: this pill for the strong, Hyeongbangpaedok-san first for the depleted, Ubanggeumryeon-tang if that fails. The claim of a hundred hits out of a hundred belongs to the genre; the sequencing by constitution is clinical thinking. **Fifty to seventy pills of a rhubarb-and-gleditsia purgative is a severe purge, and gleditsia pod is irritant and toxic in quantity. This should not be prepared.** An epidemic illness with facial swelling and clouded consciousness needs emergency care. — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
