Ginseng and Cicada Shell Powder

Insamseontoe-san

한글 인삼선퇴산  ·  Pinyin Ren Shen Chan Tui San

人參蟬蛻散

The leaf carries no indication of its own. 1.85.16 尿澁 supplies it in the clause that names this formula and the next together — 痘瘡不快煩躁咬牙尿澁宜用人參蟬蛻散紫草木通湯 — and the section's opening clauses set the frame: 痘疹小便澁者宜導赤散, and 痘毒內鬱當消息大小便如何, 如小便澁則通小便宜大連翹飮. A collation line sits inside this leaf's span in the transcription, at the page boundary immediately after the leaf: a placeholder box against 亦 for the First Edition. The character it annotates does not occur anywhere in this leaf. The only occurrence in the chapter is in the prose of 1.85.14 咬牙, two sections back — 卽咬牙也, then the unread character, then 曰戞齒 — so the note belongs there and is not printed or glossed here. It is reported to the prose worker who holds 1.85.14. 蟬殼 reaches the herb dictionary as a registered alias of 蟬退; 紫草茸 has no headword and is entered as 紫草 with the shoot recorded in the prep, which is how the corpus has handled it in eight other places.

Indications 主治

Pattern-based

  • Pox not coming out freely, with restlessness, grinding of the teeth and scanty urine (痘瘡不快, 煩躁, 咬牙, 尿澁)
  • Pox toxin pent up inside, so that the water does not pass (痘毒內鬱, 小便澁)

Biomedical correlates

  • Oliguria in smallpox — in practice the oliguria of dehydration
  • Restlessness and bruxism in a febrile child

Translation

Ginseng, cicada shell, akebia, white peony, red poria, purple gromwell shoot and licorice, 5 fun each. Cut them up, make one packet, decoct in water and take. — Gangmok 綱目 (Donguibogam · Soa)

Clinical commentary 臨床

Seven drugs in equal fives — ginseng, cicada shell, akebia, white peony, red poria, the downy shoot of purple gromwell and licorice — for a child with smallpox who is passing little or no urine, restless, and grinding his teeth. Akebia and red poria are the chapter's standing pair for driving water downward; purple gromwell is the one drug used throughout this block on the reasoning that the pox toxin has to be moved out through the blood; cicada shell is there because a cast skin was taken to help a skin eruption come out. What the physician was looking at is worth stating plainly, because the chapter gets the sign right and the cause wrong. A child with a confluent pox eruption loses a great deal of fluid through a broken skin surface, drinks badly because the mouth and throat are ulcerated, and vomits; the urine goes down because the circulating volume has gone down. 1.85.16 reads the scanty urine as toxin pent up inside and tries to open a channel for it. The scanty urine was real and it was a danger sign, and the correct answer to it — then as now — is fluid, which is the one intervention that would have changed outcomes in this block and which this formula is not. A diuretic action, had akebia and poria possessed one worth the name, would have made a dehydrated child worse rather than better. **The patient is a child and this leaf gives no dose scaled to age or weight — 5 fun of each of the seven drugs, one packet, whatever the child's size.** None of the seven is a poison, so the risk here is the risk of the thing not done rather than of the thing given. One caution belongs to akebia and not to the formula: the drug name 木通 has covered more than one plant, and material sold under it has repeatedly turned out to be Aristolochia manshuriensis, which contains aristolochic acid and is nephrotoxic and carcinogenic. True Akebia is not that plant, but the substitution is common enough that no modern preparation of a 木通 formula is safe without botanical identification. — Eui Young (Justin) Chung, L.Ac., Dipl.
