Two Yellows Salve

Ihwang-go

한글 이황고  ·  Pinyin Er Huang Gao

二黃膏

The croton is fried in the oil and then discarded, so only what it gives up to the oil remains in the salve; it is entered without grams, the leaf counting the kernels rather than weighing them. 藥水 is the 洗藥方 of this same section.

Translation

Treats every malign sore. Three nyang of clear oil; fry twenty croton kernels in it, and when they are slightly blackened take the beans out. Put in 1 nyang of beeswax and let it melt, then put in powdered sulphur and realgar, 1 don of each, and mix evenly into a salve. Wash the sore with the medicated wash and then rub the salve on two or three times; wonderfully effective. — Seui Deukhyobang 得效 (Donguibogam · Jechang)

Clinical commentary 臨床

An oil-and-wax salve in the standard proportion — three parts oil to one of beeswax — with the active drugs put in last so the heat of the melt does not drive them off. The croton is used as an infusion rather than an ingredient: the kernels are fried in the hot oil until they begin to char, and are then taken out, leaving their irritant oil behind. Sulphur and realgar are stirred in as powders. The two yellows of the name are sulphur and realgar. Sulphur is the one drug here with a defensible topical action, being a scabicide and a mild keratolytic; realgar is arsenic disulfide and was used for its supposed killing of the worm in the sore. **Contains realgar, an arsenic sulfide, and croton oil. Arsenic is absorbed through broken skin and accumulates; croton oil is one of the most severe skin irritants known and is a tumour promoter in laboratory use. Applied to an open sore and rubbed in repeatedly, this preparation is actively harmful. This must not be prepared.** — Eui Young (Justin) Chung, L.Ac., Dipl. O.M.
