Donguibogam 東醫寶鑑

治冬月手足皸裂作痛. 淸油五錢, 慢火煎沸, 入黃蠟一塊再煎, 候熔入胡粉, 五倍子末各少許, 熬令紫色爲度. 先以熱湯洗患處, 火上烘乾, 用藥付上, 以紙貼之, 其痛立止, 入水亦不落. 《得效》

Translation

Treats painful chapping and fissuring of the hands and feet in winter. Clear sesame oil 5 don, brought to a simmer over gentle fire; add one lump of beeswax and simmer again; once melted, add small amounts of white lead and powdered Chinese gall, and cook until it turns purple in colour. First wash the affected place with hot water and dry it over a fire, then apply the medicine and cover with paper. The pain stops at once, and it does not come off even in water. — Deukhyo 得效 (Donguibogam · Su)

Clinical commentary 臨床

White lead (胡粉) is lead carbonate and must be omitted — it is absorbed through broken skin and there is no safe topical dose. The remaining three ingredients make a serviceable occlusive salve on their own, with Chinese gall supplying the astringent tannin that draws fissure edges together. The application protocol is worth keeping intact: wash in hot water, dry thoroughly, apply, then cover — the same sequence a modern dermatologist gives for soak-and-seal.
