Donguibogam 東醫寶鑑

治血分. 桂皮ㆍ赤茯苓ㆍ當歸ㆍ川芎ㆍ赤芍藥ㆍ蓬朮ㆍ三稜ㆍ桑白皮ㆍ檳榔ㆍ蒼朮ㆍ大腹皮ㆍ瞿麥ㆍ靑皮ㆍ陳皮ㆍ甘草 各五分, 葶藶ㆍ大黃(煨) 各二分半. 右剉, 作一貼, 入薑五片, 水煎服. 《直指》

Translation

Treats the blood aspect. Cinnamon bark, red poria, angelica, chuanxiong, red peony, curcuma, sparganium, mulberry root bark, areca, atractylodes lancea, areca husk, dianthus, green tangerine peel, tangerine peel and licorice 5 pun each; lepidium seed and rhubarb (roasted) 2.5 pun each. Cut up, make one packet, add 5 slices of ginger, decoct in water and take. — Injae Jikji 直指 (Donguibogam · Bujong)

Clinical commentary 臨床

Seventeen medicinals at 5 pun and under — the smallest doses in the chapter — with sparganium, curcuma, angelica, chuanxiong and red peony giving it a blood layer no other oedema formula here has. The premise stated at 2.8 is that in this pattern the blood has itself turned to water, so the water is reached by moving the blood. The red swelling of the limbs that marks the blood aspect, as against the pale pitting of the qi aspect, remains a useful distinction: swelling that is warm and discoloured suggests venous or lymphatic obstruction rather than simple fluid overload.
