Each is powdered separately and the powders must not be mixed. For each dose take 1 don of each, except areca at 2 don. At the first watch before dawn, take them stirred into a strong scallion-white decoction; by mid-morning malign matter is brought down resembling fish jelly. For the vacuous, halve the dose. Follow with thin gruel, and afterward a decoction such as Cheongyeolsaseup-tang. **Note on modern practice: kansui (甘遂) is a drastic purgative and a toxic medicinal, restricted in contemporary practice. Recorded as classical text.**

Donguibogam 東醫寶鑑

治遠年脚氣難治, 此方特效. 卷柏(東向者佳, 先以鹽水煮半日, 卽甘冷水煮半日焙乾)ㆍ黑牽牛子頭末ㆍ甘遂ㆍ檳榔. 右各爲末, 不得相雜. 每服每件各取一錢, 惟檳榔二錢. 五更初, 濃煎葱白湯調下, 至辰巳時, 取下惡物如魚凍. 虛人減半服. 隨喫淡粥, 更服湯藥, 如淸熱瀉濕湯調之. 《得效》

Translation

Treats leg qi of long years that is difficult to treat; this formula is especially effective. Spikemoss — east-facing is best; first boiled half a day in salt water, then half a day in fresh cold water, and baked dry — black morning glory seed first-ground powder, kansui, and areca nut. Powder each separately; they must not be mixed. For each dose take 1 don of each, only the areca at 2 don. At the first watch before dawn, take them stirred into a strong scallion-white decoction; by mid-morning malign matter is brought down resembling fish jelly. For the vacuous, halve the dose. Clinical commentary 臨床

The chapter's answer when years of treatment have failed, and the instructions are unusually exact: powder each herb separately and do not mix them, dose before dawn, expect the result by mid-morning, halve it for a weak patient, and follow with gruel and then a gentler decoction.
