Donguibogam 東醫寶鑑

治痰迷心竅, 或似鬼祟. 半夏 一錢, 赤茯苓ㆍ天南星ㆍ枳實 各八分, 陳皮ㆍ黃芩ㆍ黃連ㆍ白朮ㆍ瓜蔞仁 各五分, 䓀莄 四分, 人參 三分, 甘草 二分. 右剉, 作一貼, 入薑 三片, 棗 二枚, 水煎, 臨臥, 入竹瀝, 薑汁調服. 先吐去痰, 次服此藥. 《入門》

Translation

Treats phlegm confounding the orifices of the heart, or a condition resembling possession by spirits. Pinellia 1 don; red poria, arisaema and bitter orange 8 pun each; tangerine peel, scutellaria, coptis, white atractylodes and trichosanthes seed 5 pun each; platycodon 4 pun; ginseng 3 pun; licorice 2 pun. Cut up, make one packet, add 3 slices of ginger and 2 jujubes, decoct in water, and at bedtime stir in bamboo sap and ginger juice and take. First induce vomiting to remove the phlegm, then take this medicine. — Uihak Immun 入門 (Donguibogam · Damum)

Clinical commentary 臨床

The section this belongs to is one of the most consequential in the Bogam. It argues that presentations read as possession — deranged speech, disordered perception, behaviour the family cannot account for — are phlegm in the heart orifices, and Dongwon's line quoted alongside is blunt about the stakes: where blood and qi are both vacuous and phlegm has lodged in the middle burner so that the twelve offices each lose their function, treating the patient as though an evil had entered will kill them. That is a fourteenth-century argument for a natural cause against a supernatural one, and for treating the person rather than the perceived intrusion. The formula follows the reasoning. Ginseng, white atractylodes and jujube sit inside an attacking prescription because the etiology names vacuity as the opening. The instruction to induce vomiting first and give this afterwards makes it the second step of a two-stage treatment, not a standalone. **Acute confusion, deranged speech or altered consciousness requires urgent medical assessment.** These presentations overlap with encephalitis, metabolic derangement, drug effects and acute psychiatric emergencies, and this passage describes a historical framework rather than authorizing management outside conventional care. — Eui Young (Justin) Chung, L.Ac., Dipl.
